Available Forms
Individual Acknowledgment
This form is used when a signer is acknowledging that they willingly signed a document for its intended purpose.
This form is used when a signer is swearing or affirming that the statements in a document are true.
Verification on Oath or Affirmation
Copy Certificate
This is a certificate used when requesting a certified copy of an original document, where permitted by state law. In Georgia a Notary Public can not certify a copy of any Georgia Vital Records (birth certificates, death certificates, etc.)
